Organise observations, difficulties and questions for yourself. A local worksheet without an account, diagnosis or transfer of your entries.
Your notes stay with you.
This worksheet does not send entries to us or other services and does not save them permanently. Closing or reloading may erase them. Use your own protected device for sensitive notes.
You do not need to record every sensation. Once or twice a day, briefly note what mattered in daily life. Good days and helpful situations count too.
Choose no more than two or three observations, such as sleep and feeling hot. Describe their effects in your own words. If possible, change only one small everyday habit at a time. This can reveal patterns but does not prove cause and effect.
Observe for fourteen days — no score and no diagnosis
