An appointment is easier to use well when you have thought about your main questions beforehand. This planner helps you collect them and record what was agreed. It does not create a treatment plan or interpret your entries medically.
Your tool
This tool does not send your entries. They remain in this open page.
Begin with everyday life
Describe what you hope to improve: perhaps moving more comfortably, managing meals or understanding treatment options. A specific situation can explain more than a target weight alone. You do not need to prove success or present a perfect history. A short, honest description gives the conversation a useful starting point.
Put questions in priority order
Write the question you would least like to leave unanswered first. Add two others. You might ask about expected benefits, harms, alternatives, costs or follow-up. If time is limited, your care team can help decide what to cover today and what needs another appointment. The list is there to support a conversation, not to rush through a checklist.
Bring your medicine information
Include prescribed medicines, products bought without a prescription and supplements. Copy names from packaging or an existing medicine list rather than guessing. Your notes do not replace a professional medicines review. If details are missing, bring the packaging or a current list. Do not change anything you take simply to complete this worksheet.
Record agreements clearly
Use the final field during or after the appointment. Note who will do what, how to ask further questions and when a review is planned. Ask for unfamiliar explanations in everyday language. Before leaving, try summarising: have I understood correctly that I should gather these documents first and then contact the practice again?
Look after the notes
Entries remain in the open tool and are not saved automatically. Reloading may lose them. The print function lets you create paper notes or a PDF; the planner cannot protect that downloaded file. On a shared device, collect printed pages, manage saved files carefully and clear the fields afterwards. Avoid including identifiers that are not needed for your own preparation.
Common questions
Can the form send notes to my practice?
There is no sending function. Ask the practice which secure route it uses for receiving documents.
Must I complete every field?
No. The fields are optional memory aids. Clear priorities matter more than collecting as much information as possible.
