A prescription does not tell you who will pay the bill. If you are considering a weight-management medicine in Germany, separate four questions: Is it approved, is it appropriate for you, can you obtain it, and does your insurance cover this use?

What statutory health insurance excludes
Annex II of the German medicines directive lists Wegovy and Mounjaro as excluded from statutory coverage when used for weight reduction. The exclusion concerns the purpose of treatment. It does not mean that obesity is merely an appearance issue or that professional support is unnecessary.
This research was checked on 16 October 2026. The linked consolidated annex carries an amendment date of 20 August 2025. A further decision from September 2026 was still marked as not yet in force on the official decision page. The rule in force at the time of your decision is what matters.
Why diabetes creates a different coverage question
Mounjaro is also approved for treating type 2 diabetes. A prescription for that purpose needs its own assessment. Someone else’s pharmacy receipt therefore tells you little about your own entitlement: their diagnosis, authorised use and insurance arrangements may be different.
Having diabetes as well does not automatically make every weight-management treatment reimbursable. Ask the practice to explain the intended purpose of the prescription. A vague statement that “this ingredient is sometimes covered” is not a useful answer to your individual funding question.
Approval is different from availability
The current EMA overview includes injectable and tablet forms of Wegovy; tablets are intended for adults. That does not establish that every form is stocked by German pharmacies or paid for by insurance. An announced launch date is not a stock check either.
Ask the pharmacy about the exact product, prescribed form and pack you need. Establish how long a quoted price remains valid. Questions about the appropriate treatment should not be decided simply by choosing whichever pack appears cheaper at that moment.
Build a complete cost picture
A realistic budget includes more than the medicine itself. Ask what consultations, follow-up and additional support are planned, and which services are charged separately. Use confirmed quotations rather than a precise-looking online price list that may already be out of date.
- Medicine: product, pack, quoted price and quotation date.
- Practice: initial consultation, follow-up appointments and possible investigations.
- Support: for example, agreed nutritional counselling.
- Time period: distinguish the price per pack, per month and per planned phase of care.
Private cover or self-funding: clarify before committing
A private insurer will assess the contract and the proposed care. Ask for a written answer that refers to your policy rather than interpreting general advertising as approval. Submitting a receipt after purchase is different from confirming funding beforehand.
If you will pay yourself, discuss whether ongoing costs are manageable. Ask how your care could be supported if money or supply becomes a problem. Cheaper alternatives need discussion with the treatment team; stretching intervals, splitting doses or changing medicines yourself is not a budgeting strategy.
Prepare your funding discussion
- Write down your insurance arrangement and the exact medicine discussed.
- Request an understandable list of the planned services and charges.
- Clarify unresolved coverage questions in writing before making financial commitments.
- Record confirmed amounts and their dates in your budget.
Common questions
Does a private prescription mean the treatment is unreliable?
No. Medically appropriate care can fall outside statutory insurance benefits. A careful assessment, an understandable plan and transparent charges matter. Equally, a private prescription by itself does not prove that a particular medicine is suitable for you.
Can I compare tablets and injections by monthly price alone?
Cost is only one part of the comparison. The form, way it is taken, approved patient group and clinical assessment may differ. Compare options that are genuinely available and have been discussed for your circumstances, including which services each quotation covers.
